• Nominate from: October 1 to October 31, 2026 at 11:59 p.m.
• Award recipients will be recognized at the December 7, 2026 Medical Staff Quarterly Meeting.
New annual awards for PHC Medical Staff
Any PHC Medical Staff member may nominate an eligible colleague.
Note: PHC leaders or self-nominations are not eligible.
STEP 1 | NOMINATE A COLLEAGUE (SUBMIT ONLINE)
• Nominate individuals in one of the three awards categories above.
• Write up to 300 words describing the nominee’s contributions and impact in the award category.
• Obtain endorsement from another Medical Staff colleague.
• Gather the nominee’s title, department, site, email address.
• Submit by October 31, 2026 (Link available here on October 1)
STEP 2 | COMMITTEE REVIEW: NOVEMBER
The Award Committee independently evaluates all nominations using a standardized scoring rubric.
• Three assessment criteria
• Weighted scoring system
• Total score out of 100 points
