NEW | ANNUAL AWARDS FOR PHC MEDICAL STAFF
Do you have a medical staff colleague whose contributions exemplify compassionate care, collegiality, and meaningful impact beyond clinical practice? Nominate them for a 2026 Medical Staff Award of Distinction!
Award recipients to be recognized at the Dec. 7, 2026 Medical Staff Quarterly Meeting.
Any PHC Medical Staff member may nominate an eligible colleague.
PHC leaders or self-nominations are not eligible.
STEP 1 | NOMINATE A COLLEAGUE (Easy online submit)
• Choose a colleague to nominate for one of three awards
• Get endorsement from another Medical Staff member
• Gather name, e-mail and department for the nominee, you, and the other supporting member
• Prepare up to 300 words describing the nominee’s contributions and impact in the award category you choose
Submit online by October 31, 2026. Thank you!
STEP 2 | COMMITTEE REVIEW: NOVEMBER
The Award Committee independently evaluates all nominations using a standardized scoring rubric.
• Three assessment criteria
• Weighted scoring system
• Total score out of 100 points

• One recipient from each category will be formally recognized at the December 7 Medical Staff Quarterly Meeting.
